Background
A Sri Lankan patient, accompanied by his cardiologist brother, presented with a challenging case involving the lower terminal molar. Radiographic evaluation revealed complex anatomy and high procedural risks.
Challenges:
- Lower last molar
- Long roots
- Multi-dimensional curvature
- Deep previous cavity margins from previous restoration
- The torque between the root canal walls and files was very strong and possibility of file fracture was high.
The key distinctive management for this case are:
- Obtaining glide path to a K-file size 20.
- Very frequent irrigation and recapitulation.
- Not forcing the file and removing it after 3-4 strokes.
- Using a more tapered, flexible, yet cutting endodontic files.
